CONSENT RELEASE FORM

I the parent/guardian of [Child Name], and I hereby consent to release my child to the care of LifePointe Church for the Kids bash on July 17 & 18, 2025.

I understand that LifePointe Church will take all reasonable precautions to ensure the safety and well-being of my child during the event, including but not limited to providing adequate supervision and following all applicable laws and regulations.

I acknowledge that there are inherent risks involved in participating in the event, and I accept full responsibility for any injury, loss, or damage that may occur as a result of my child's participation.

I hereby give permission for LifePointe Church to take and use photographs or videos of my child for promotional purposes, including but not limited to use on their website or social media channels.
